Health Insurance

  • Disabled Dependent Form: Anthem’s Disabled Dependent Form helps members secure continued coverage for eligible dependents, providing important care for loved ones who need extended support.
  • Anthem's Special Offers: Enjoy extra perks through Anthem’s Special Offers, which provide access to exclusive discounts on health-related products, services, and wellness programs.

 

High Deductible Health Plan and Health Savings Account (HSA) 

  • High Deductible Health Plan (HDHP): This Anthem HDHP covers 100% of preventive services with no out-of-pocket costs. For non-preventive care, members pay up to a deductible—$2,000 for individuals and $4,000 for families—after which in-network care is fully covered.
  • HSA Guidance for Ages 65+: The Health Savings Accounts for People Approaching or Over Age 65 Brochure provides essential information for older employees on managing and maximizing HSA benefits as they approach retirement, ensuring financial preparedness for healthcare needs.
  • Additional Contributions & Resources: Employees 55+ can add $1,000 to their HSA. HSA Bank offers resources, webinars, and calculators to manage contributions and expenses.

 

HMO Site of Service Health Plan

  • Anthem HMO Site of Service (SOS) Plan: Similar to a traditional HMO but includes specialized SOS components for labs, imaging, outpatient surgery, and therapy services with designated copays.
  • SmartShopper Incentives: Members can earn incentives by using SOS-approved locations. Use the Anthem website or call member services to find eligible sites and schedule services.
  • Away from Home Care: Anthem offers this program for HMO members needing healthcare coverage while away, such as college students out of state. Check the participating states booklet for coverage areas.
  • Anthem Out-of-State Plan: Designed for retirees residing outside of New England, this plan ensures comprehensive healthcare coverage across state lines, tailored to meet the unique needs of out-of-state members.

Dental Insurance
