Life Insurance
The City of Manchester offers the UNUM Life Insurance plan to all employees (working 20 hours or more per week) and their dependents. The City has an automatic Basic Life Insurance benefit tied to to an employee's salary, capped at $50,000 in coverage. The Supplemental benefit is voluntarily enrolled, managed, and paid by employees, who are accountable for covering 100% of the associated expenses. The costs will be deducted in payroll. To file a claim for any of the voluntary benefits with UNUM, please call 1-800-635-5597.
Supplemental Life Insurance Amounts:
- Empoyee Coverage: $2,000 to a maximum of $300,000
- Evidence Of Insurability is required: if selecting over $150,000 for yourself or over $20,000 for your spouse
- Submit the EOI form at: SecureHealth.Unum.com/EOIAccess (Access code: 4TW4R7M)
- Spouse Coverage: 1/2 or less of the employee's coverage amount (up to a maximum of $150,000)
- Child(ren) Coverage: $10,000 for each child up to age 23 (Age 24 & 25 if FT Student)
Assigning Beneficiaries
Both Basic and the Supplemental Life Insurances require you to elect beneficiaries. Please utilize this Beneficiary Form to specify them and send it to HR.
A beneficiary is the person who receives financial benefits from an insurance policy when the insured person dies. There are two types: primary and contingent. If the primary beneficiary is unable to receive the benefit, it goes to the contingent beneficiary. Payouts can be split among multiple beneficiaries, totaling 100% for each.

Long Term Disability
The City of Manchester offers UNUM Long Term Disability to all employees (working 30 hours or more per week). The City has an automatic Basic Long Term Disability Insurance benefit tied to the employee's salary, capped at $100,000 in coverage. The City's insurance covers up to 24 months (2 years), while this optional Supplemental Long Term Disability will cover the employee after the 24 month period is up until their normal Social Security Retirement Age.

Basic and Optional Long Term Disability provides coverage that pays you a benefit of 60% of your earnings to a maximum monthly benefit of $5,000 per month.
